2013 Enrolments / Re-enrolment Week

Enrolment of children takes place one week each year for current and new clients.

Current Clients Re-enrolment week commences: Monday 21^st October 2012 starting at 7am

New Clients Enrolment week commences: Monday 28^th October 2012 starting at 7am

Enrolments are based on available spaces.

The following procedures will apply:

• All enrolment forms MUST be filled out completely prior to handing it in otherwise it will not be accepted. We will NOT photocopy immunisation records. All forms must be fully signed and your Health Centre Record Book must be sighted on the day by staff.
• Your direct debit form for EziDebit must be completely filled out with all bank details and attached to your enrolment form.
